A written request from your solicitor is required before an assessment can be booked. This request must contain the following information:

• Client’s name and date of birth

• Assessment request (i.e., The type of report required) 

• Location (i.e., Prison and name of the facility, or community)

• Date that report is due and details on chances of adjournment 

• Name and contact details of the solicitor 

• Return of signed consent form 

• Including collection of collateral from past or present treating clinicians and public or private healthcare services
